Description
As part of the fix for GMT 157, we need a new UI page for the service desk to use to insert new POPs into gmoc-db, based on information site admins provide as part of http://groups.geni.net/geni/wiki/GENIMetaOps/SiteCredentials.
Create this new UI page, and verify that the POP entries it creates are sufficient for gmoc-db data submission.

Until this ticket is resolved, the workaround will be for GMOC to manually insert new POP entries into the database when the service desk adds new sites.
